...is a bit vague on one point. Would it be enough to simply transition socially, without getting surgery, or would surgery be required in order to serve as the opposite gender? If the former, I see problems ahead, especially for MTFs. I imagine certain cis female service members might object to a pre-op or non-op MTF sharing quarters with them.

It is not going to be easy, and I'm not sure they've thought it all out. I addressed what I thought were significant issues in Soaring With Eagles. The other problem is acceptance among the military. The colonels will call their meetings and tell their subordinates to follow the rules, to not harass the transgender. All the briefings and all the rules will not eliminate transphobia. Just like the mandates and briefings in the early 1970s did not eliminate racism in the military.

To be practical I think surgery will be required

At least for front line troops where becoming a POW becomes a distinct possibility, how does one ensure they are treated correctly? Above and beyond the usual difficulties of course. And yes I would think in close quarters such as submarines and the like it might cause complications.
